Choose your main question

An appointment can cover a lot of ground. Write down the question that matters most to you and a few details you want to remember. You do not need to arrive with a diagnosis or specialized vocabulary.

Describe your situation

Explain when a change began and what you have noticed. Bring a list of the products or medicines you use when relevant. Your clinician can decide which details matter and what further questions to ask.

Clarify the next step

Before you leave, ask what the next step involves, when to follow up, and who to contact with questions. Ask for clarification if instructions are unclear. Keep private health information out of public subscription and contact forms.
